A copy of your USA Hockey Registration Form with bar code and confirmation number (Please Note: you must do this electronically on line with USA Hockey).  

 

For spring registration you can reprint USA hockey registration

Follow the easy registration process and then print a copy of your

registration confirmation sheet with bar code and get it to your team manager as soon as possible.  You can email the confirmation sheet as an attachment.
